For over a century dentists treated tooth decay by removing both diseased portions of the tooth and healthy structure deemed at risk for future decay. In the 1970s, though, a new approach emerged, known as Minimally Invasive Dentistry (MID). This practice protocol attempts to preserve as much of the healthy structure as possible.
Before MID, dentists followed a decay treatment protocol developed in the 19th Century. A part of this became known as extension for prevention calling for dentists to remove healthy structure considered vulnerable to decay. Besides reducing the tooth's volume, this practice also resulted in, by today's standards, larger than necessary fillings.
It was thought that removing this additional material would make it easier to clean bacterial plaque, the source of decay, but later, research showed the practice couldn't guarantee the teeth wouldn't be reinfected.
Since then we've learned a lot more about teeth and have developed new ways to detect decay at earlier stages. X-ray imaging, for example, has transitioned largely from film to digital technology, providing more detailed images at greater magnification. This, along with laser fluorescence and infrared cameras, has made it easier to detect the first tiny stages of decay.
We can also limit tooth decay damage by boosting enamel strength with fluoride applications and sealants or reducing decay-causing bacteria with anti-bacterial rinses. We've also seen advancement in techniques like air abrasion that remove decayed tooth material while leaving more healthy structure intact better than using a traditional dental drill.
Restoring teeth after treatment has also improved. While dental metal amalgam is still used for some fillings, the main choice is now composite resin. These new tooth-colored dental materials require less tooth preparation (and thus less material loss) and bond well to the remaining structure, resulting in a stronger tooth.
Following a MID protocol leads to less intervention and less time in the dentist's chair. It also means preserving more of a natural tooth, an important aim in promoting long-lasting dental health.
October brings fall leaves, pumpkins — and National Dental Hygiene Month. As you change your summer clothes for a fall wardrobe, it may also be time to change your toothbrush for a new one. The American Dental Association (ADA) recommends replacing your toothbrush every three to four months. If that sounds like a lot, just think: This small but very important tool gets a lot of use!
If you brush your teeth twice a day for two minutes each time as recommended by the ADA, that’s two hours of brushing action in one month. Three to four months of twice-daily brushing makes for six to eight hours of brushing time, or a couple hundred uses. This is all an average toothbrush can take before it stops doing its job effectively.
Toothbrush bristles are manufactured to have the right amount of give, tapering, and end-rounding for optimal cleaning. When new, a toothbrush can work its way around corners and between teeth to remove dental plaque. Old bristles, however, lose the flexibility needed to reach into nooks and crannies for a thorough cleaning. Worn bristles may curl, fray or break — and can scratch your gums or tooth enamel. A toothbrush with stiff, curled bristles does not leave your mouth feeling as clean. This may lead to brushing too often or too hard, which is bad for your gums.
A good rule of thumb is to replace your toothbrush every season — unless you see signs that you need a new one sooner. For example, if you wear braces, you may have to replace your toothbrush more frequently since brushing around braces puts more wear and tear on the brush.
For healthy teeth and gums, make sure your primary oral hygiene tool is in tip-top shape. Taking care of the little things now can avoid inconvenient and expensive dental problems later. The scientific evidence is overwhelming that sugar is the most important dietary factor in causing tooth decay, and soft drinks are the largest source of sugar in many kids' diets. Even natural fruit juices have unhealthy amounts of sugar. So when it comes to beverages, keep it simple: H₂0 is the way to go!
Tip 2: Get Creative With Shapes & Flavors
Healthy food that's low in sugar doesn't have to be boring! You can use cookie-cutters to shape calcium-rich cheese and whole-grain bread into flowers, stars — even dinosaurs. Unbuttered popcorn can be flavor-boosted with a dash of cinnamon or parmesan cheese. There are all kinds of ways to get creative.
Tip 3: Sweeten The Deal With Fruits & Veggies
While fruits and vegetables do have some sugar, they are a good choice for a healthy smile — and a healthy body. That's because they also contain plenty of water and fiber, which slows the body's absorption of the sugar… and even helps clean the teeth! Kids enjoy the naturally sweet taste of bite-sized fruits and vegetables like cherry tomatoes, baby carrots and seedless grapes. And the cheerful, bright colors of these nutritious little nuggets make them even harder to resist!

When you’re among the top players in your field, you need every advantage to help you stay competitive: Not just the best equipment, but anything else that relieves pain and stress, and allows you to play better. For top-seeded Canadian tennis player Milos Raonic, that extra help came in a somewhat unexpected form: a custom made mouthguard that he wears on the court and off. “[It helps] to not grind my teeth while I play,” said the 25-year-old up-and-coming ace. “It just causes stress and headaches sometimes.”
Mouthguards are often worn by athletes engaged in sports that carry the risk of dental injury — such as basketball, football, hockey, and some two dozen others; wearing one is a great way to keep your teeth from being seriously injured. But Raonic’s mouthguard isn’t primarily for safety; it’s actually designed to help him solve the problem of teeth grinding, or bruxism. This habitual behavior causes him to unconsciously tense up his jaw, potentially leading to problems with muscles and teeth.
Bruxism is a common issue that’s often caused or aggravated by stress. You don’t have to be a world-class athlete to suffer from this condition: Everyday anxieties can have the same effect. The behavior is often worsened when you consume stimulating substances, such as alcohol, tobacco, caffeine, and other drugs.
While bruxism affects thousands of people, some don’t even suspect they have it. That’s because it may occur at any time — even while you’re asleep! The powerful jaw muscles that clench and grind teeth together can wear down tooth enamel, and damage both natural teeth and dental work. They can even cause loose teeth! What’s more, a clenching and grinding habit can result in pain, headaches and muscle soreness… which can really put you off your game.
There are several ways to relieve the problem of bruxism. Stress reduction is one approach that works in some cases. When it’s not enough, a custom made occlusal guard (also called a night guard or mouthguard) provided by our office can make a big difference. “When I don’t sleep with it for a night,” Raonic said “I can feel my jaw muscles just tense up the next day. I don’t sense myself grinding but I can sort of feel that difference the next day.”
Â An occlusal guard is made from an exact model of your own mouth. It helps to keep your teeth in better alignment and prevent them from coming into contact, so they can’t damage each other. It also protects your jaw joints from being stressed by excessive force. Plus, it’s secure and comfortable to wear. “I wear it all the time other than when I’m eating, so I got used to it pretty quickly,” said Raonic.

Teeth-replacing dental implants not only look life-like, they’re made to last. For one thing, the metals and dental materials used in them are impervious to bacterial infection.
But that doesn’t mean implants are impervious to failure. Implants depend on the bone and other natural mouth structures for support. If the bone becomes weakened due to disease, the implant could become unstable and ultimately fail.
Peri-implantitis, the condition that can lead to this kind of failure, is a major concern for implant longevity. It’s a type of periodontal (gum) disease triggered by plaque, a thin film of food particles that can build up quickly in the absence of adequate brushing and flossing. The gum tissues around the implant become infected and inflamed.
If the infection isn’t properly treated with renewed oral hygiene and clinical plaque removal, it could spread below the gum line and begin to damage the underlying gum tissues and bone. This could destroy the all-important connection between the titanium implant post and the bone. The implant could eventually loosen and become completely detached from the bone.
The key is early intervention before the bone becomes damaged. Besides plaque removal we may also need to apply antibiotics in some form to control the growth of disease-causing bacteria. If the disease has fairly advanced we may also need to consider surgical repair to strengthen the attachment between implant and bone.
You can help to avoid peri-implantitis altogether by practicing consistent daily brushing and flossing around all your teeth including the implant, and seeing your dentist at least twice a year for cleanings and checkups. And by all means see your dentist if you notice any signs of gum swelling, redness or bleeding. Staying on top of your gum health will help not only the natural tissues and remaining teeth in your mouth, it will help preserve your implants for decades to come.
